Investigators have opened a fourth case regarding the site of Massachusetts’ Wynn Boston Harbor resort and casino after a construction worker died in an accident last week.

What Happened

At around 2:45 p.m. last Tuesday (April 3) reports say that Joe Teixeira, 56, was struck by a piece of heavy machinery while operating an excavator on the construction site. Witnesses say he was taking apart a trench box when something fell onto the cab of the excavator.

Teixeira reportedly regained consciousness at the scene, but died later at Massachusetts General Hospital.

The Boston Globe reports that Teixeira was a four-year employee at J. Derenzo Companies, a subcontractor on the $2.4 million project headed by lead contractor Suffolk Construction.

“It is with tremendous sadness that we learn of the loss of long-time J. Derenzo Co. employee and friend, Joe Teixeira, resulting from injuries sustained at the Wynn Boston job site,” Derenzo said in a statement. “Our deepest-felt thoughts and prayers are with Joe’s family, friends and co-workers, as well as everyone impacted by this incident.”

Wynn, local and state police, as well as the Occupational Safety and Health Administration, are looking into the incident, and the man’s official cause of death has yet to be determined.

OSHA reportedly has three other open investigations on the site associated with previous accidents.

The Boston Herald reports that last month, a worker was injured when a scissor lift tipped over; in January, a worker fell down an elevator shaft; and late last year, a worker was hit by an excavator bucket.

The project, with up to 1,500 workers on site daily, is the largest private construction development in state history. The resort and casino is slated to open to the public in June 2019.
