Faulty workmanship can be overcome by effective Quality control on the job site. May add cost but will also save money down the road.

Agreed, Humans can never be perfect and the only way to minimize risk is to put process in place to check and re-check workmanship.

Improper surface preparation is the best answer, but it can be included in faulty workmanship.

In supplying house paints to the DIY market there are two major contributing factors to paint failing ... 1) the seller of the paint failing to give clear and concise information on how to apply the coating and 2) the customer not following that advice and taking shortcuts to save time or money in the short term.

I agree with g. KV. Surface preparation is the biggest problem but that should also come under Bad Specifications and Ineffective QC.
