Improper/insufficient surface prep is so vague and all encompassing that it really needs to be broken down if the information/response is to be of use to the reader.

says it a;; for me, in all the times I have seen coatings failure, everyone has been due to poor surface prep, be it not cleaning or lack or sufficient abrasion, or whatever the applicator has been asked to do, but thinks they can not bother because you can't see after application
