Lack of effective supervision and poor training are leading causes of accidents. Workers do not own or control the workplace: employers do. Employers must clearly establish safety policies and procedures, teach them to the workers and enforce them, those who do not follow the rules can be subjected to discipline,...all in the same order.

Poor training, poor enforcement, lack of awareness, damaged and incorrectly maintained equipment, lack of proper anchorages, there are far too many things to go wrong! Throw in cost cutting and incompetence and you have a recipe for a disaster! Accidents are only a specific combination of events that when aligned cause the whole pack of cards to collapse, most the time if it takes ten things to make an accident happen, we are running with nine aligned so it seems that luck plays more than a little part, frightening really!

It is my experience that the foremen are the worst offenders. Working with no safety equipment at all. I also fault the customer from not stopping work and insisting that safety policies be followed. It is a sad situation. For those of you who do this, shame on you.
