Great article!!! Lets all remeber you can have a confined space plan or a safety manual 6" thick , however all are useless without irob clad implementation. Otherwise it is all window dressing.

Lack of supervision and effective training are probably two of the most common causes of accidents, including confined space entry. The regulations are in place to prevent accidents, but the enforcement is lacking and the penallties for violators of rules are laughbable. Paying fines for serious or fatal accidents seems to be part of doing business; mandatory incarceration for willful negligence should be a powerful deterrence, yet to be implemented.
